Corrective Actions

At any time during the semester, if a student demonstrates an unsatisfactory behavior, the student will be informed and the behavior documented. The instructor will write a description of the infraction in detail. Contents of a written corrective action are: date written, type of corrective action (as described below), statement of the specific performance problem(s), reference to previous counseling about the particular performance problem(s) if appropriate, a definitive statement of the performance expectations, and action plan for resolving the problem, and any standard by which improvement will be measured. This will be signed by the instructor and student involved. If the student refuses to sign, the Program Director will assign a witness to sign, indicating the student's refusal to sign.
